STEP 1

'E KOMO MAI (Welcome)

At check in, our medical assistants will measure your blood pressure and BMI. They’ll also perform a retinal scan or update your labs if needed. Our point of care lab can measure your A1c and microalbumin in just 7 to 10 minutes.

STEP 2

PONO (Make Right)

Our clinical pharmacists from Times Pharmacy will review the medications and supplements you are currently taking. They will make any necessary adjustments to help you better manage your diabetes.
STEP 3

ALOHA (Compassion)

Our nurse practioner will review your medical history, perform a medical exam, and evaluate your lab results. She will also regulate your medications, examine your feet, and review your personalized care plan.
STEP 4

HO'OHOLO (Self Control)

Our CDE-RD will discuss your current diet, make recommendations to improve your diet, and work with you to schedule both online and face-to-face nutrition classes.
STEP 5

HO'OMALU (Peace)

Our medical social worker will discuss ways to utilize resources you can use to help you better manage diabetes. They’ll connect you with everything from transportation services to behavioral health services.

24/7 ACCESS TO CARE

We’ve partnered with Vivante Health to offer you round-the-clock access to care and online resources through Vivante’s virtual portal. Vivante will provide you with a digital blood glucose log, access to online education, and access to a virtual care team.

Digital Blood Glucose Log

Track your glucose levels using Vivante’s app – available on phone, tablet, or computer. Manually enter your glucometer readings into the app, or take advantage of bluetooth compatibility to send your results directly from your glucometer to the app.

Online Education

On-demand online education is available through Vivante’s app. These sessions are designed to compliment the face-to-face education sessions that you will participate in at the ‘Ekahi Health Center.

Virtual Care Team

Vivante has a team of nurses, pharmacists, certified diabetes education providers, and medical social workers that are available around the clock for all your diabetes needs.

Additional Services

Group Patient Education Sessions

We offer small group education classes to help you understand diabetes and the steps you can take to better manage diabetes. These include nutritional educational classes, exercise and stress management classes, and support groups for participants who have been diagnosed with diabetes.

Behavioral Health Services

Our licensed mental health counselors and licensed social workers are available for one-on-one and family counseling sessions.

Holistic Health Services

Take advantage of our holistic services such as reiki, reflexology, massage therapy, and acupuncture.
