COVID-19 (Coronavirus) Health Annoucement

For New Patients: (Current Patients please scroll to the bottom for instructions)

Due to COVID-19 (Coronavirus), we are no longer offering physical, in-clinic visits at ‘Ekahi Health. We have since turned our clinic 100% virtual as we continue to offer you the same experience via telehealth. We are currently using the InputHealth platform’s virtual visit feature to help us connect with you wherever you may be. We will also be utilizing Zoom for our hui group education sessions when they resume. We can also connect with you over the phone.

If you are scheduled for an initial intake appointment for our ‘Ekahi Diabetes Management Program, please follow the following steps:

1. Please review our “Insurance Financial Policy & Privacy Practices” document located below on this page.

2. Please download, fill out, and digitally sign the forms in the document titled “New Patient Signature Forms.” This is a fillable PDF, so you do not need to print, you can fill them out on your computer.

3. Please email these “New Patient Signature Forms” along with photos of your picture ID and insurance cards to our office manager, Mahealani, at mtaitaguelaforga@ekahihealth.com. If texting the photos is easier for you, text our scheduling line at (808) 224-8966. You can also choose to use fax instead: (808) 447-0571.

4. If you can, please print the “Intake Info Packet” (also provided below) or have it up on your computer screen during your scheduled intake appointment. We will be reviewing this document during the appointment.
